When designing evaluation studies, we can use two different methodological methods – prospective or retrospective. However, both prospective studies took into account the hypothesis formulated a priori, and retrospective studies based on the posteriori hypothesis, contain limitations as to the degree of certainty of the recommendations made.
A prospective approach
A prospective approach should be adopted, we are dealing with requirements similar to experimental conditions. We are preparing an original therapeutic or preventive program, we qualify groups of patients for it and we provide access to resources, select the application in life. The specific problems available as part of the analysis of the results are associated with the heterogeneity of the cases of use of the analyzed analyzes, which can significantly affect the course of treatment and its effectiveness. In various medical research, in the field of young drugs, the research project cannot cover these homogeneous groups due to specific socio-demographic parameters.
And although from a methodological point of view it is an optimized solution that includes the selection of a homogeneous group in terms of such features as age, sex, place of residence, family situation, selected drug use, but taking into account ethical factors, we can not use the therapeutic program adopted e.g. only boys aged 18, addicted to amphetamine function for two years, relationships with families full and residing in a big city.
First of all, when choosing a group for a therapeutic program, we cannot make such selection results. Secondly, you can guess what the use of clinical trials is, such a homogeneous group would treat itself much worse than a heterogeneous group. In this case, the effectiveness of treatment of drug addiction is associated, in whole, also with a large variety of therapeutic group in terms of the age of patients, their advanced addiction or used drugs.
It is also important that patients come from different backgrounds. Experiences with Polish therapeutic centers, where the casino of patients tried to “force” people from only one province, are definitely negative. The drug addicts environment is used in the same way as drug and criminogenic connections, patients, even if they are used with different, though not too distant cities, most often not only know each other, but also often took drugs together or traded with them. Undoubtedly, it expands to those undergoing treatment during the so-called drug addiction, group division services for abstinence from alcohol or even drugs.
How to use therapeutic experience, beneficial for the course of therapy is that they are not related in one group and do not participate in international drug research, used at different ages and addicted to different drugs.
